Volunteer Agreement & Guidelines
I agree to the following terms and conditions, intending to be legally bound by them:
1. I will abide by the mission, rules, regulations, policies and programs of The Haven.
2. I will stay with my child/children and supervise his/her activities while volunteering if under 18 years of age.
3. We will promptly return all The Haven's supplies, equipment, records, moneys and other items in good, clean condition as requested.
4. I assume the risks of me and my child/children being bitten, scratched, injured, or frightened by cats, kittens, dogs or puppies or other animals while volunteering for The Haven.
5. The Haven is not liable to me for any injuries, damages, liabilities, losses, judgements, costs or expenses whatsoever which I, my child/children, may sustain while volunteering for The Haven. I will indemnify, defend, and hold The Haven harmless from and against all claims, lawsuits, injuries, damages, losses, costs or expenses whatsoever, sustained by any companion animals or any person in connection with my intentional misconduct or grossly negligent performance of volunteer activities for The Haven, or my breach of The Haven's rules, regulations, policies and programs.
6. I understand and agree that The Haven may refuse a volunteer application for any reason.
7. If I foster an animal, I consent to The Haven visiting my home or business from time to time to observe the animals and their living quarters. I will bring the animal to The Haven for adoption events or if requested by The Haven.
8. I have accurately and truthfully completed this Application and Agreement.
9. I will treat animals with respect and kindness and agree to use safety precautions.
Guidelines
Volunteers are required to:
1. Attend interview, orientations and training as The Haven deems necessary.
2. Actively volunteer within a six month time frame before being taken off the call list. All volunteers must fill out a new volunteer application every year.
3. Be polite, helpful, courteous and respectful of animals and people.
4. Adhere to rules explained in training for animal handling.
5. Refer questions of public regarding The Haven's policies to staff only.
6. Maintain confidentiality in regards to clients, donors and business associates.
7. Accept supervision by professional staff person responsible for volunteer activities.
8. Volunteers are responsible for keeping track of their volunteer hours.
9. Notify The Haven if are unable to work your scheduled shift as early as possible.
10. Wear appropriate attire and follow no smoking rules. Volunteers are encouraged to purchase a Haven T-shirt to wear at Haven events.
11. Only be in areas of The Haven with proper training and supervision.
12. Get permission before bringing a guest.
13. Agree to The Haven's right to release volunteers for unsafe practices, for not following procedures, being unproductive and for inappropriate behavior.

14. Report any accident or injury to either person or animal to The Haven staff.

By submitting this application, I affirm that the facts set forth in it are true and complete. I understand that if I am accepted as a volunteer, any false statements, omissions, or other misrepresentations made by me on this application may result in my immediate dismissal.

Volunteer Code Of Conduct
At all times, The Haven volunteers shall:
1. Refrain from making any derogatory or nagative statements about The Haven, other organizations or individuals, and the staff.
2. Not represent The Haven to the media without prior approval of The Haven director.
3. While volunteering, wear a Haven T-shirt.
4. Accept assignments as directed by The Haven Volunteer Coordinator and/or the director.
5. Refrain from speaking to members of the media without permission from the director.
6. Give all donations received to a director, volunteer coordinator or other Haven staff.
The Haven reserves the right to suspend or deactivate any volunteer for breach of this code of conduct.

(No applicant will be denied the opportunity to volunteer solely on the grounds of conviction of a criminal offense. The nature of the offense, the surrounding circumstances and the relevance of the offense to the postion applied for many, however be considered.)
Waivers
Tetanus Information: I understand that because I, or my child, may handle animals it is important to discuss the tetanus vaccine with my physician. I release The Haven from all responsibility that may occur because of my not pursuing this matter further and I understand whatever decision I make is at my own rist. I have read, understand and agree to the above tetanus information.
In the event of an emergency, I hereby give The Haven permission to seek medical attention for myself or my child.
I give permission for The Haven to photograph or videotape or record me or my child for use in any Haven publication, educational or advertising purposes The Haven may designate.
I acknowledge and understand that as a volunteer of The Haven I, or my child, is not covered by The Haven's workers compensation or any other insurance policy for any damages or injuries I, or my child, may sustain during volunteer activities.
Any modification to this agreement must be in writing and signed by both parties. This agreement is binding upon The Haven, me, and the Haven's and my respective heirs, successors, assigns, executors and personal representatives.
